If you are making a Life Cover or Life Income Cover claim please provide the following:
- A copy of the death certificate.
- If the cause of death is noted as being 'subject to Coroner's findings' we will also need a
copy of the Coroner's report. If you don't have this report, we can request it on your behalf.
- A copy of the birth certificate or passport of the deceased person.
- If the deceased person was the sole policy owner, we will need to see the Letters of Administration
and Probate, to determine the policy beneficiary. If you are unsure who owns the policy, please check
the policy schedule for the original owner and the Memorandum of Transfer for any transfer of
ownership that may have taken place. The Memorandum of Transfer is located at the back of the policy
document.

Once we have received the completed form we will gather the information that we need from your doctor
so that we can assess your claim as quickly as possible.
