Claim forms and information
We most commonly receive claims for Life, Trauma, Complete Disablement Cover as well as Monthly Benefit claims. The forms and information you need to make a claim under these covers are available below. If the insurance product you want to claim for is not listed please give us a call on 0508 464 999 and we will let you know what you need to do.
Providing a certified copy - There are a number of people you can ask to certify your documents: your adviser, doctor, lawyer or a Justice of the Peace (JP). They need to sight the original document and sign each page to certify that it is the same as the original.
